The IP address is reported using '%a'
The hostname is reported using '%A'

Try

   format1 'Testing: %a: %W Trap (%q) Uptime: %#T%#v\n'

(within the snmptrapd.conf file), and send a v1 trap to snmptrapd.

All of the data generated in your IT infrastructure is seriously valuable.
Why? It contains a definitive record of application performance, security
threats, fraudulent activity, and more. Splunk takes this data and makes
sense of it. IT sense. And common sense.
