If all you need is one-to-one communication (which doesn't meet your "up to
4 people" requirment) it is actually possible to dial an IP from a VOIP
phone (at least some of them) and have a direct phone to phone connection.
Now the phone has to be exposed on the internet, but it is not too hard to
restrict where that port is accessible from.
