   Notes for reviewers

Several commits introduce secure defaults with a named opt-out
(SendTimeoutMillis, MatchTimeoutMillis, LockTimeoutMillis); those are behaviour
changes on upgrade and are recorded in the changelog. The documentation commits
deliberately settle recurring reports in the threat model rather than changing
code.
