We have Cisco CME at each of our satellite sites and a combo of a traditional PBX and Asterisk in the Head Office.
Cisco is very expensive and far from perfect. Asterisk is very flexible and cheap. The commercial products make it easier for mass phone deployment (just plug in the phone and it updates and configures itself).
