Open source efforts/software like OpenMRS, WorldVistA (VistA Office etc.), OSCAR etc. that are focused on diffusion/uptake and continuous improvement. All need to have practical tools methods etc. to work effectively in the heterogeneous health IT ecosystem. Building on Tim's view:
