Occasionally when the active controller is suddenly pulled from the system the 
standby fails to take over and actually reboots its node.  The following error 
logs are seen.

Any thoughts on what might be occurring here?  Also what is the ‘state’ being 
referred to - can it be viewed using amf-state or immlist ?
