Definately the latter - he should be dressed while having this talk.
Might it be possible for you to offer them both a one-to-one talk
with you, and tell them a day or two before, so they can prepare
questions also? I'm sure you have time limits (which I luckily don't
have to worry much about in my current job). You could tell them
it's very usual to have a few worries and concerns, that are best
expressed one-to-one - just so they don't worry if something is
wrong.

BTW - a client of mine has hit his carers more than once while
they helped him with showering (long ago now). Too little room
and too little dignity is a dangerous combination, I think....
