There are some "ROM" modules (really firmware modules) running around 
with Flash chips on them.  They probably weren't from production runs 
though.  I'd guess they were internal Apple development tools used 
for quick testing of firmware revisions.   Still, they've managed to 
"escape" into the hands of non-Apple folk.

I've seen modules with four, eight and even sixteen chips.  The 
sixteen chip module had a little XOR gate to handle the CE so that 
they could be used in two banks--high address and low address.
