I use MacDialup.com. For $9.99 I get all I need. The only thing that bothered me at first was sometimes when downloading, the Bit(or is it Byte) rate varied wildly & sometimes slowed down to the point where IE's Download Manager cut it off. As soon as I called them about it, they switched me to another access number which fixed the problem. I use a PM 7500 w/an 8500 mobo upgraded to G3/400, 56k modem.
