Dave - If you go to our website (www.computerneedsinc.com), select the Forms tab. There are 6 samples there that I created using OmniForm a couple of years ago. Select the form you want to look at using one of the 6 buttons on the left and the click the "PDF link". That will launch the pdf version of the form. There is no intelligence behind the fields - just fill and print. I've had people launch in Reader 4.0 and above and haven't heard anyone complain yet. Have you tried to use the created PDF with Acrobat, Adobe Approval, or Adobe Reader?
