Just use the right tools.
Mac OS X's PDF generator (Quartz) is a "record and playback system", meaning that whatever you feed in is what you will get out. Same is true, to an extent, with Distiller. If you start with RGB colors, you will get RGB in the PDF.
This means that if you want a CMYK-based PDF, you will need to work in an application that knows about CMYK - for example, Stone Create.
Preflight tools like Pitstop refuses them systematically...

And some RIP don't support OpenType fonts
Given that PDF doesn't support OpenType fonts, that's not an issue. Mac OS X, like Distiller, does the correct thing of "converting" OpenType to either Type 1 or TrueType when creating the PDF.
