At this point we are using Acrobat's built in launch index command under
the Document Properties>Advanced tab. I'm not JavaScript savvy so the
concept never occurred to me. I was hoping that this problem would have been
obvious to Adobe and the solution would have been provided.

We author all pdf content on Macs and hence if it is broken there I wouldn't
even bother to check Windows. Thanks for the info. I was unaware Acrobat 5
on a Mac couldn't launch an index either.
