The RIP vendors/hardware manufacturers require the commands to be prepended to the files - thus rendering (no pun intended) them illegal as PDFs - but the various RIPs consume them as correct non-the-less.
We didn't invent this workflow, we just have to live with customers who use it. If you really want to experience horror, understand that the alternative in use at some shops is to embed PostScript into the PDFs as PSObjects and to control the device with setpagedevice commands.
Normally, the prepending of comments happens "just" before the job goes into the RIP (usually by some automated system) so that no one ever has to try and open the file with Acrobat... right.
We have added support in our products a long time ago for both the PSObject setpagedevice and prepending of %%comments forms of device control by request from both customers and hardware manufactures.
I guess its just reality PDF...

You can prepend these commands to a PDF file with a binary copy in dos:
copy header+foo.pdf output.pdf
or you can do it with simple OPEN/CLOSE/READ/WRITE commands.
I have to throw up my hands in horror! That makes a PDF that is incorrect, according to the terms of the PDF Reference. The fact that Acrobat accepts it doesn't mean other consumers will, or even should...
