I'm obviously doing something wrong either in the creation of the ZIP
file or the repo.xml.

Don't set the defaultState to disabled (in install.xml), but enabled. It won't be installed unless the user decides to do so. And why install if you don't want to enable it anyway?

This might actually be a bug nobody has discovered yet, as this kind of configuration doesn't make much sense.

When you change this, then please make sure you bump the version number. Otherwise there might be problems with caches etc.
