If I guess what Robert is asking about, forms are not a good answer. If PoDoFo ends up too awkward or heavy to do simple mail-merge style customization of the PDF (so that the "Dear Mrs. Smith" in the PDF is not then editable by the end user), I can provide a 100-line perl script that I developed for my own use.
