I don't think it's an SMTPUTF8 issue.

SMTPUTF8 primary handle non-ascii local parts. Michaels is 'mjt' ...

It's more about encoding the From header's displayname.
I think, Michael expect that postfix' sendmail proper encode non-ascii GECOS 
when using them as displayname.

That would eliminate all SMTPUTF8 requirements.
