How have past distrributions handled breaking changes in newer
versions of Berkeley DB?
Pretty much the same. Major releases of EL avoid breaking changes by backporting bug and security fixes for the life of the release and thus staying on the same version of BDB. If there's any breaking changes they will happen between major releases.

Very rarely a package has been upgraded in a point release that causes some sort of breaking change, but I don't recall this ever happening with Berkeley DB. It pretty much only happens when a major security issue is extremely difficult or impossible to fix with a backport.
