Postfix was released late 1998, and IPv6 support was integrated
some six years later, in 2005.
By that time the size of the README source files had grown to 31882
lines, and I am not surprised that not every relevant piece of text
was updated to mention whether an IPv6 address must be enclosed in [].
My impression is that missing instances can easily be found by
scanning documentation for text that talks about matching an IP
address, /file/name, or type:table.
