This could be because: 0) The sender is <>. 1) The sender sent mail with "RCPT TO: <address> NOTIFY=NONE". 2) You are using a Milter application that adds a recipient with "NOTIFY=NONE". 2.5) I vaguely recall that old Postfix milter clients always used "NOTIFY=NONE" when adding a recipient, but I may be mistaken. 3) You are using an older Postfix implementation that always used "NOTIFY=NONE" when adding a BCC recipient.
