It made me wonder about headers manipulation so I have few questions:

- are you aware of email servers that can/will manipulate headers so that recipients order is altered?

No.

I mean, not any pure MTA. It might be done in integrated mail systems like CommuniGatePro or Zimbra, where a web MUA interface is used to compose mail, but even there it would be incidental to constructing headers initially, not an intentional reordering.

One could implement this in a milter, of course, and I've even written code for a MIMEDefang installation that takes care to NOT reorder addresses in address list headers when normalizing them (to avoid having Sendmail 'fix' them after signing.)

- would it be possible to randomize recipients order with some header rewrite in Postfix?

Not directly in Postfix. Wietse is not actively and intentionally evil.
