if the always_add_missing_headers is "no", postfix adds headers only
when client belongs to local_header_rewrite_clients.
the default for local_header_rewrite_clients is "permit_inet_interfaces"
which means, the headers are added when you submit mail from the same
machine..
try from another IP address.
