Do you require a written record from each licensee, NRC Form 5, for the current year when personnel process in? NO 3. Do you utilize PADS when the new individual does not have a dose estimate? YES, we use PADS always unless the data is not in PADS then we use a "Current Year Dose Information" form 4. Approximately how many man-hours are spent on processing one person in; including time it takes person processing in to provide documentation? Anywhere from 5 minutes to 20 minutes 5. How many persons, techs and professional staff are involved in your dosimetry program during outages and normal operations?
