1)       No lifetime, just current year, we print up the NRC form 4 and
attach it in but are only concerned with current year since there is no
lifetime limit anymore

2)       If we don't have their exposure in PADs then yes, we require a
Form 5 or equivalent for that current year

3)       Yes we utilize PADs- if it says nothing, we still attach that
page on to show they haven't gone anywhere

4)       Anywhere from 5 min to a day in extreme cases where the
individual is close to their dose limit and records aren't in PADs

5)       Outage- 4-5 Normal operations 1-2

STP would like to know the following Dosimetry Benchmarking Questions:

 

1.      Do you require a detailed lifetime exposure records, NRC Form 4,
when personnel process in?
2.      Do you require a written record from each licensee, NRC Form 5,
for the current year when personnel process in?
3.      Do you utilize PADS when the new individual does not have a dose
estimate?
4.      Approximately how many man-hours are spent on processing one
person in; including time it takes person processing in to provide
documentation?
5.      How many persons, techs and professional staff are involved in
your dosimetry program during outages and normal operations?  Include
TLD issuing and collection, whole body counting, dose records
maintenance, processing personnel TLDs, etc.
