STP would like to know the following Dosimetry Benchmarking Questions: 1. Do you require a detailed lifetime exposure records, NRC Form 4, when personnel process in? No. 2. Do you require a written record from each licensee, NRC Form 5, for the current year when personnel process in? No. 3. Do you utilize PADS when the new individual does not have a dose estimate? Yes, for the current year only. 4. Approximately how many man-hours are spent on processing one person in; including time it takes person processing in to provide documentation? Approximately 5 to 10 minutes. 5. How many persons, techs and professional staff are involved in your dosimetry program during outages and normal operations? Include TLD issuing and collection, whole body counting, dose records maintenance, processing personnel TLDs, etc. One person non-outage with periodic support during TLD exchange, Personnel exposure investigations, ED maintenance, fit testing, training and vacation/holidays.
