1.      Has your PWR facility undergone a power up rate?  YES
2.      If so, has a neutron characterization study/evaluation been 
performed post design mod?  YES
3.      If so, did the measurements conclude a neutron correction factor 
variations or program changes?   DATA CONFIRMED THE CORRECTION FACTOR THAT 
WE USED FOR PORTABLE INSTRUMENTS.  DATA WAS USED TO DERIVE TLD NEUTRON 
RESPONSE CORRECTION FACTORS - PRIOR TO THIS, OUR TLDs WERE NOT USED FOR 
NEUTRON MONITORING.

remain signed in all day?

BWR - No it is not required, control room and chem lab / offices are 
within the RCA, therefore those folks end up wearing an ED all shift. For 
other on shift personnel most are allowed to wear ED to facilitate 
response to emergencies within the RCA.

No it is not required. Most are required to sign out after each entry. The 
only exception is for Operations, Chemistry and RP if they are going in 
and out frequently of the RCA. If they are going in and C-zone, high rad 
or LHRA they sign out each time. We have emergency EDs for emergency 
responders
