Callaway is requesting benchmark information on your Radioactive Source
Control Program to include the following questions:

1. Who (title) provides overall oversight of the control of Radioactive
Sources (day to day oversight, not the overall "responsibility", such as
RPM).

2. Who (title) performs receipt surveys.

3. Who (Title) performs classification of source for Licensable Status.

4. What database is used (locally grown such as "Excel or Access", or do
you use a commercial product, and yes, I'm aware of Benjy's benchmark
question last spring)

5. Do you have any "built in" Human Performance Tools (Self Check, Peer
Check, etc.) to your process of classifying the source and/or entry to the
tracking program.
