RP Instrumentation/Dosimetry First Line Supervisor 2. Who (title) performs receipt surveys. Task Qualified RP Technician 3. Who (Title) performs classification of source for Licensable Status. Health Physicist 4. What database is used (locally grown such as "Excel or Access", or do you use a commercial product, and yes, I'm aware of Benjy's benchmark question last spring) We use an Access database 5. Do you have any "built in" Human Performance Tools (Self Check, Peer Check, etc.) to your process of classifying the source and/or entry to the tracking program. RP Inst Tech inputs data. RP supv validates the data input then signs receipt documentation.
