I'm a bit concerned about all the requests for the PHI "mapflow." Mapping
your data flow refers to the process each covered entity should go through
to determine a number of different items as part of that entity's
assessment of its compliance with HIPAA . When an entity maps the data
flow it identifies where, when, and how PHI enters the entities, who
receives the PHI, where, when, how and with whom the PHI is shared within
the entity and where, when, how and to whom the PHI is disclosed outside
the entity. All of this is particular to the agency. For those CEs that
do the same thing, another's data flow map can be helpful. But the entity
must be sure to do its own data flow mapping.

I would love to receive a copy of your map flow as well. We have
been working to develop a questionnaire that can be used to identify
how PHI flows through a department. It is tough to nail this down so
it makes sense in all situations.
