By far, the biggest "hit" (running just Q-S) is likely due to the overhead of the perl interpreter and the perlscan body/attachment searches.
RE: AV clamscan overhead is likely too high for high-volume use. You could use a client/server AV ... e.g. sophie/trophie (but not free). clamd is available but hasn't yet been fully integrated with Q-S.
RE: SpamAssassin
Not knowing your SA config, you could tweak things like turning off
RBL, avoiding Razor, not doing MX lookups, adn other network ops that add time/overhead. Of course your results may suffer a bit.
However, regardless of minor tweaks to AV and/or SA, it looks like Q-S itself is going to be your biggest issue.
