What does the end of /mnt/qtp-sandbox/usr/src/qtp-upgrade/log/build-recent.log 
say?
Almost definitely this is because of missing/misversioned perl RPMs.
I know that the build scripts are no longer the "supported" method of getting 
QMT up and running, but its the only way I've ever felt comfortable with the 
installs 
(http://www.qmailtoaster.org/old/centos/cnt40/EZ-QmailToaster-CentOS-4.3.txt).
Also, if you've used vqadmin to update your server, you're probably in trouble. 
Its fine for browsing around and reading info about your server, but it does 
not properly configure the server. qmailadmin seems to work fine for the 
handful of things I've used it for though (aliases, mailing lists, and turning 
on the spam folder).

You have to watch your logs. Use 'tail -n 100 /var/log/qmail/*/current | 
tai64nlocal | less' for a quick look via command line, or search the list for 
my comments about splunk. Splunk is nice because you can watch an email flow 
through the system in just short of real time.
Of course theres a zillion other things... just a couple bullet points to think 
about.

Various things seem to be happening with my QMT-ISO install. Since I've run the 
newmodel update, I've not been able to complete it and things aren't working 
well anymore since.

For example, I'm getting up to 4 copies of the same emails at times. Email is 
taking minutes to hours on receive and send at times. Possibly other problems 
since I don't seem to be getting the same amounts of email I was getting before 
either.
