You don't show how you have configured your <Realm> - could you send your
complete configuration file please?

Also note that Cisco's require a Service-Type = Framed-User and 
Framed-Protocol = PPP reply items (these should match the contents of the
Access-Request from the Cisco).

If you still have a problem please send a trace 4 debug showing what happens.
