My "dirty" trick for otherwise incurable brake squeal, after trying all the
things you listed, is dirt. Throw a handful at the pads, making sure some
gets between the pads and the rim, and take it down a hill. Mud puddles
also work well. That'll "machine" the sidewalls pretty quickly.
