This is very close to what I do, except that for multi-disc sets, I
place the individual disc folders below the album folder.

/Flac/Artist/Album/Disc 1
/Flac/Artist/Album/Disc 2

Same here.  Larger might be better, but it's often hard to find larger
artwork.  I much prefer to download artwork rather than having to scan
it myself.  I also place a 1 pixel black border around the artwork,
which I think looks better, so the process is usually to crop and
resize to 298x298, then add the border.

What do you need to keep track of?  The SqueezeCenter database is a
pretty good starting point.

I don't think there's any reason to place these files in a
subdirectory, and it might interfere with SqueezeCenter or other
software from seeing the album artwork.  Also, I use Burnnn with the
non-compliant CUE sheets that I generate in EAC and it expects the
audio files to be in the same folder.  So I store the following all in
an album folder:

- Flac files
- EAC generated non-compliant CUE sheet
- EAC ripping log
- cover.jpg
- any other artwork files

If you do this, just go to SqueezeCenter Settings > Advanced > File
Types and add the 'cue' extension to Disabled Audio File Extensions.  I
add it to both 'disabled' fields just to be sure.
