Just for the test can you copy the EXE (and the INI) to c:\ and test it that way?

Yes - I have specified the path to SMTP2POP3.exe within the command-line. Which is:

C:\Program Files\Salive\external\SMTP2POP3.exe ini="C:\Program Files\Salive\external\ahmed.ini" section=local

The application [smtp2pop3.exe] is located in the path specified as shown below:

SMTP2POP3 is not specificly testing the SMTP server, but tests the mailflow.
 
If you mean that the entry with the SMTP2POP3 external check is seen as down, then I would like to know what logging you get in Servers Alive for that entry.
(also check if the timeout within Servers Alive is at least the time of the INI file, so in your example at least 100)

It appears my setup for smtp2pop3 is wrong;

1. Loads of mails are generated and sent to the POP3 mailbox - yet Salive reports my SMTP server is down.

2.  It doesn't delete mail in the POP3 mailbox as warned in the document.

What value should I place on the External check tab - e.g should it be equal to 1 or what? I have tried all possible values but no luck.

Below is the heading of the mail sent to the pop3 mailbox

From: joebloggs.org.uk
To: pop3usr@xxxcom
