Thanks! That was the clue Dirk.  Looks like someone must have changed the service account settings and had turned off 'interact with desktop'. I'd tried changing to another account, but forgot to add the desktop interaction parameter.
 
All is working now.... I knew I'd be successful finding help through the SAlist.

SA service?  hmm though you said application.
If it's the service, then you'll only see the GUI IF the service is running as system account with desktop interaction AND you're looking directly on the system running the service (that is NOT via a normal TS session).

Hi Dirk, Telephony was running. I stopped SA, and then TAPI, restarted TAPI, then SA service, but no change.

The fact that you see it as .ex instead of .exe is a limit of the task manager, nothing more.
Check if the telephony service is running before starting Servers Alive.

Hi, I've been running SA for a few months now, but just came back from vacation to see that my monitoring box was down.  I brought it up, and see that SA doesn't appear on the GUI, nor the system tray. FromTask Manager, I can see a process running "serversalive.ex"  Note. the file suffix is "ex" not "exe" which seems suspicious.  I upgraded from 4.1.1591 to 4.1.1609, but still have the same problem.  The program comes up if I run from the start menu.
 
Any ideas?
