RE: [Care2002-developers] AIDS ARV ART with Care2x

2004-09-02 Thread Mark Painter
Hi

There is a huge need for ARV HIS in South Africa and probably in many other
places in the world. Our area is about to pilot a commercial program
(web-based, Oracle, Java).

Care2x has all the building blocks required for ARV - what is required is a
specific front end GUI. There would also need to be some specific tables
added (WHO classification, etc).

IMHO Care2x is too flexible (if that can be possible) for the average user,
especially for something like ARV therapy.

Is any-one interested in helping me to create the ART module?

I could help with some design and UI issues, but don't have time or skill to
help with coding.

[Care2002-developers] Care2x for OPD Use!

2004-09-02 Thread Nyawallow James
Hi Everyone,
This is my first post coz I believe I need to make a decision at this
juncture but first some background... I have been toying/beta testing in
the IT Room care2x since the 0.* days with a view to adopt it as HMIS
(hospital management information system) for a Non profit and the ONLY
children's hospital in my country Kenya( and in East Central Africa!).
During those times I had volunteered as the Hospital Sysadmin ..I still
am on Part-time non paid basis. The Hospital is called Gertrude's Garden
Children Hospital (www.gerties.org ...not well done site but was again a
volunteer work) Back to the main Issue. I have been testing Care2x due
to the perceived benefits..Open source et al. and the Hospital Admin
(CEO) is in the picture and in agreement totally as I wanted it
implemented as the ONLY HMIS system at the main hospital and the
branches (2). The Main problem I have is I do not know any clear cut way
to use for especially OPD (Out Patient department) (the hospital has
both in(about 80 beds and growing)/out patients) but the majority is OPD
averaging 500-600 per day.
My fix I am finding difficulties on how a patient who makes an OPD visit
goes through workflow until discharge! It sounds silly even to me but a
Patient here gets registered first, then sees a nurse who takes the
basic patient measurements (temp/pressure..), then send the patient to
see a doctor who then recommends tests (x-ray/lab)then makes a
prescription once the results are back then the patient goes to pharmacy
to pick medicine then discharged/admitted for IP! Could someone give an
equivalent flow within Care2x for this? (U can do this off the list if
necessary) **This is specialist Children's Hospital but care2x seems to
be for Adults Mainly

My other issues are Pharmacy, Billing and Laboratories, Apparently the
Lab Manager and techs don’t seem to understand the tests in Care 2x! Are
these std? The other issue is we only have one Lab therefore the
splitting of labs into central etc does not make sense to them but I do
not know how to consolidate the lab to one single unit. However I
currently trying to understand closely how most the procedures flow so
that I can prepare a training manual for the Clerks/nurses/doctors etc,
Any help will be highly appreciated as I really desire to see this work
at the hospital which (The Hospital IT Mgr) would like to use as a test
case for the country and demo for other hospitals in the country.

2004-09-02 Thread Roel De Meester
I follow you , for doing research you should always collect as much data
as possible and never throw away 'trivial' data in advance.
But in case of HIV Therapy followup, the 10 institutes/centres that i
know (and for which i would like to develop an ARV module) do not
hospitalize the HIV infected people.
The aim of the institutes is to follow up the patients, give them
appropriate therapy and keep them from getting AIDS.
Besides that they do clinical studies, collect data, do research etc...
Mind.. I'm not a doctor. I'm a humble software developer who happens to
know some people in these institutes and i helped them to setup a simple
database from which they can query some data. I'd like to help them out
by merging there current way of working into Care2x to take benefits
from both.

Remember this if you get too focused you will loose capibilities you
will need a future period of time. Especially when you start tracking
true stastices related to patient care.

For instance it may seem superfical to track patient beds, equipment,
but when you have to start looking for the cause of a VRE or MRSA out
break in your aids patients you will ned this data because this
equipment can become the means of spreading the desease from patient to
patient.

You should collect the data and trim how you present the data to the end
user, but you should always collect the data inorder to leave your
research options open.

Main conclusions about Care2x to support ART

Advantages:
It runs on a modern web thin client web server platform and is all open
source software.
It will run on any hardware platform. It is scalable and can run large
enterprises. Open source will let you maintain and modify the software
to meet your needs.

Disadvantages:
This is a large software package with many functions that may not be
necessary in smaller hospitals and may interfere with effective training
and use.
Although this software can be used for outpatient EMR, it is not
designed for that use. The extra features have the potential to cause
confusion when they are not used.
This software will require modification to support ART.

The main issue is that Care2x is development to process almost all
information in a large scale clinic, while for ART the most important
things are
1. Only Out Patients.
2. Main users are a)Administration[check-in], b)(Social)Nurses,
c)Doctors, d)Bio-statisticians

Will the Care2x PM solve these issues?
What is the current status of this 'branch'? Where are we now on the
