yes but we must never believe the computer practices medicine.  systems 
are often scape goated for people not doing their jobs double checking, 
and assessing the patient.  Even if it is the right drug the patients 
condition might change as such it is the humans responsibility to adjust 
for the change in condition not the computer.

URGENT get back with me. I want to make the disaster database active 
that we donated earlier this year.  Need to put a team together to get 
this done.  I think we should make it available to all our ASIAN 
developers.  It is perfect for this disaster.  We did calculations early 
on found this system was capable of collecting millions of records if 
the network was deployed properly. 

For members who have not seen the package it is at www.pcrx911.com, 
current user name matthew pass word atropine.
PatientCareRx has been working on this for some time and we are going to 
give it away opensource through the GPL.  Now I just need a programmer 
to finish off the barcode generation section, and additional servers.  
For now I will be available to host on my t-1. 

Granted it is a development package but it is needed now. Lets get it done.

Emir,
Axis cameras are nice but steep priced for general documentation looking 
in the 75-125 range.  My vision is that when documenting the care 2x 
user would open the database to where the image will be collect there by 
creating the relationship to the image and topic of that documentation.  
This method would assign the image name, date, time, and store the user 
id of who took the picture automatically.  The user would then point the 
camera at the wound, bruise, etc.  in the java applet the user would see 
the image the camera is collecting in preview form inorder to position 
the camera where needed. The user then would snap a high resolution 
still shot hit enter an the image would be collected. 

This would allow the user to then go back to the data base pull up 
images documenting the same injuring over time for comparison rapidly.

Ok I will write the specs on how it should work.  This is very 
important.  Because the whole point here is to limit work load, increase 
effieciency, and make images a standard part of documentation.

Phase one we need to get  a standard USB web camera to work through 
java.  Starting there  I think the crew needs to research the hardware.  
The hardware should work on linux, windows, and mac so we have the 
greatest coverage.

Specifications will be written by the end of the week.

after the forms are done, rapid charting by grid image is done, and we 
have happy customer that is next on the list.  I want the elpido to get 
his guys to donate if possible the controls package.  I have some really 
cool ideas for you to do with them will. send out line late next weeks.  
Lets just say the form will name and archive the data automatically, you 
will then use java to put together a comparative imaging program.  This 
goes beyond just telemedicine, this is in daily use digital diaganostics.  

Note the rapid charting image solution is essential for my concept to 
work with this efficiently.

Wearable computer (WC) is also not more than a design study. The only
purpose is to make any announcement which might positively influence on
stock quotes of the involved companies (infineon e.a.). They try to use the
first mover advantage. Presumably there already are taking place heavy
patenting activties in this area. May be industry will get blocked totally
in these activities, so we will not see WC within the next 20 years. BTW:
Who would like to wear a thick jacket, weighing several pounds inside the
hospital ? BTW2: To my opinion wearable computing is total fake: It is so
easy to attach some press-buttons to a piece of tissue.
What is really state-of-the-art is to be seen and studied -as always and
ever- in the military area: Did you guys see any soldiers equipped with
wearable computers in the TV ?? (Yes they have wireless communication build
into their helmets..but else ? Is this good for their health ?)
To my opinion, it will take several years before we see real-world
applications on the market for wearable computing. And, again: this race
will be won by power technology more than by computing technology.
I do not even know of any bluetooth-application which was officially
certified for medical use. On the contrary: hospital managers tell, wireless
technology is not even a serious issue in potentially life-threating
environments, like hospitals.
I am really surprised how easy-believing the community seems to be towards
fairy tails from shareholder-industry.

personally I think Sony has a better product than OQO.  Check the U70.
It's still too big for the whitecoat pocket and pricey (2600 USD) but
you can tell they're heading in the right direction.  Also, far as I
know the OQO isn't for sale yet as its a prototype...

I have tested some wearable computers for our projects most are not 
designed to be worn and people are getting into patent positions to 
prevent competition and growth.  Many companies are sue happy if you 
disparage their products, so I will not name names.  I will only say 
that the technology is there if people were to be serious about making 
the technology rather than hyping themselves at trade shows.

Off shore work is fine.  Reasonable will be based upon experience,
productivity, and resume.  We are a start up company, and will need
programmers to aument our local programmer to accomplish specific tasks.  I

rules and protocols to adhere to when filling out a prescription
to allow this to happen and be tracked. I would think there are
other similar situations in different countries around the world.

I certainly think that securing the default ordering methods to
use proper security with certificates etc would be an advantage
no matter what your country of origin.

did not hijack anything. I tried to upload x3 three never got it working.
easier to send snail mail since I am a new father and dont have time figure
out this thing today.  Wilfried I am trying very hard to push Care2x in
america, and am sharing data that I recieve as fast and freely as I can.  If
you have problem with my hijacking a thread or anything else let me know
and I will decide accordingly whether Care2x is right for me.  I neither
have the time or the patience to deal with personality issues have a baby to
raise and business to run. I appreciated the offer to upload I tried ealier
and was going to try again tomorrow when I had time, but in the mean time I

Daniel,

I think communication between doctors is essential also but we may need to
decide what format this should occur in.  Especially in light of the HIPPA
laws in the states this has become my biggest headache.  Some sort of tiered
communciation structure with the patient having ultimate authority whom
their record is shared with for consultation is what is needed and what the
HIPPA law in the states tries to enforce.

Elpido does it have to be a email or is some other type of encrypted
messaging system possible, and could this be more efficient?

This function that the doctors can change messages with each others is
essential, but if the actual module is a problem, let's change! A web
based email is a very good choose or maybe we can use too, something
like instant messages applications which is easier to use.

I know it has been months since we last posted to the list, after much 
rangling and work we have gotten our first customer and intend to deploy 
care2x make modification for our needs then send those modification on 
the reset of the developers for consideration. 

My initial customer will operate a hospice and palitive care institute 
for home bound patients.  We will treate this a vitual hospital which 
makes care2x a perfect solution.  Additionally we will be adding home a 
home based telemedicine system to the mix in the very near future for 
both healthly and unhealthly patients creating a virtual housecall.

Last we talked I wanted to make our PCRx911 disaster database available 
for possible integration but got side tracked when one of our few 
windows machines caught fire then got infected with a virus after 
rebuild.  Please tell me the preferable method for making this available 
as tar or other file type for developers to see the back end.

Will need to hire developer programmers to customize this program for 
our needs anyone interested please contact Matthew Lottinger asap.

Some bug reports when I try to up date the currency it will not accept 
the US dollar into the data base, constantly having to enter passwords 
is their anyway to get the system to maintain state for say 15 at a time 
allowing someone logged to freely navigate during that time period, how 
do I change the look of the cite to say institute powered by Care2x., 
how do I generate barcodes for products in inventory, and fianlly on the 
barcode lable sheet what kind of lable sheet is that formatted for?

Our company intends due to hippa compliance issures to integrated smart 
card identification into the system, and possibly biometric finger 
prints in the future.

Nursing reports need a lot of work will begin on that today, and post 
format in the comming days this is necessary due to the way medicare and 
insurance is reimbursed here and our need for a digital diagnostic 
reports in the future.

Ok Elipido the following new branchs may be necessary.  The first 
problem is here in America Healthcare is very complex business in that 
cost versus reimbursement can easily destroy a healthcare institution.  
My customer needs to know the following information on an on going basis.

cost versus reimbursment interms of profit.

for instance the cost of treating a patient comprises of the follow sets 
of data that can be readily derived from the care2x patient record. 

  1. labor cost all expressed a cost based upon the hourly rate of the
 provider spent with the patient during each enconter
1. nursing
2. physical therapy
3. physician
4. nurse practicianer
  2. Medical supply usage
1. disposable medical supply cost --when used in patients chart
   this cost should be immediately charged
2. Durable medical equiment
  3. Liability insurance
  4. Optional cost
1. travel cost for home care
  5. reimbursement rate as set by medicare, medicaid, insurance, or fee
 for service
Each days services should be expressed in the form of both a spread 
sheet and pie chart per patient in the system. With weekly, monthly, and 
yearly reports being presented in the line graph form. 

Further more each individual employee needs to be presented daily and 
weekly basis in the form of a pie chart based upon their types of 
reimbursement this breaks down like this for instance
Revenue component

  1. fee for services--patients paying out of pocket
  2. medicare
  3. medicaid
  4. private insurance
 versus
   Cost component
  1. supply usage
  2. hourly rate
  3. lost time  travel, restocking, ect
  4. paper work correction for reimbursemtent and other clerical time
More financial information to be monitored to be provided next week.

branch 2 a focused nursing admittance for not a narrative.  The current 
format of Care2x leaves too much up to the nurse to document rather than 
prompting her for the necessary data for future digital diagnostic 
programs and minimum standard required of a chart.  this fortunately can 
be rapidly corrected.  Our company in conjunction with our customer will 
create this format in the next two weeks.  Then we will need to employ 
some developers to code this.
