Here's an old post: The password recovery on a LS100 is unbeliveably complicated. You have to enter a very cryptic string from a hidden menu on the LS100. I had to do it on mine, so you'll need to call TAC and open a case to get this resolved. I believe that the default password is c. (Just the letter not the quotes.) Cisco can only defend the LS100 by mentioning they didn't design it, but put their label on it. Honestly, I found things that I had to load with binary or hex switches more intuitively obvious. If, before I had seen the LS100, someone had told me that anyone would implement a command line with 14 positional parameters, with no help or editing, I would have had hysterics.
