It's easier than I thought, and discovered by accident.
Clicked on the volume in mediamonkey and it changed to edit mode (if you
open the edit dialogue, you can't change the levels)

Adjusted it way down, then up, reloaded in LMS, could see the volume
adjustment data, and hear the difference.

So, good news, MM tags do work in LMS.

The drop-down gives two choices:
/tmp/images
/ /proc /sys /dev/pts [& several others]

I do see the write-permission message.

I saved the backup file to /tmp/images, logged into the RPi4 from my
smartphone and downloaded the file, and transferred it to my desktop PC.
I then used etcher to write the image to a 16gb card. That took 30
minutes. Booted the pi with the backup card.

Yes, but that's not an issue as I'm retrieving the current coverid with
the tags:c parameter and not caching it myself so it's OK for it to be
mutable.

Yes. You will find the reference to the image cache ID in the tracks
table of the database. You can call it in other ways than
/current/cover.jpg iirc 
Bear in mind the cache reference will change on a clear and rescan

I generally use folder.jpg for cover art ... so I just tried to retrieve
the cover art for a track that I know has no embedded artwork and the
album has a folder.jpg ... and it worked as I think you would hope.

The correct artwork was shown even though the URL asks for cover.jpg

I think the following rules apply in this order
1. Use embedded artwork in first track of album
2. Use folder.jpg if it exists
3. Use cover.jpg if it exists

Exactly. I use dbpa for ripping and converting and build in adding
ReplayGain tags with the dbpa utility DSP.  4 tags are written:  Album
Gain, Track Gain, Album Peak, and Track Peak.  And LMS (and every other
program I use) uses these tags properly control inter-track volume.

You don’t need another step. Look at the DSP options in DBPA. You can
set it to write the replay gain tags as part of the rip OR (not
recommended) permanently apply the gain to the ripped files.

I have no idea about MediaMonkey tags but mine look like this. If your
files already have tags it is an easy test to do. You could ask
MediaMonkey why their tags are different to both dbpoweramp and Foobar
