Both Jari and Eric suggested "reflow systems across pages" as the possible
cause of this problem.  This box is ticked and always has been - I have
never changed it.

Is the option « reflow systems evenly » checked? I did once forget to 
check it and hat problems similar as yours.

Do you have "Reflow Systems Across Page" set in the Update Layout 
Options?

This would be a simple explanation and easy to fix, except it is not true
(for 2002a on Windows).  I always have 'automatic update layout' set and
have found a number of situations where the last system on a page is
duplicated on the next page.  I have usually had to use 'Redefine page' to
fix this problem.

This also occurs with 'automatic music spacing' where certain circumstances
seem to bypass the spacing process and it is necessary to click on a
measure, with speedy input, and then click somewhere else on the page (or
run music spacing to fix several staves at once).

I have not managed to work out what the real cause of either of these errors
is yet.

Yep, I hear you! The key command for Update Layout needs to be as 
automatic as blinking for you, and the last thing you do before 
printing. Sorry for the bad news.

I think this is a throwback to the older days when screen redraws took a 
lot of time and really slowed things down.  With this, assuming you 
could keep things straight in your head, you could move things around on 
screen and when you were finally ready to "set things in stone" you 
could hit ctrl-u and get everything moved exactly as you had wanted.

It is something I learned early on -- each time I go to page view I hit 
ctrl-u, each time I move things around in page view I hit ctrl-u, each 
time I add anything in page view I hit ctrl-u (well, actually, if I am 
adding a bunch of things to a single page I will work on that page until 
I think I am finished and then I will hit crtl-u.)

And while a lot of the computers that we use these days don't have the 
slow screen-redraw issue any longer, I would bet that removing that 
behavior would be a programming nightmare.  They even added Automatic 
Update Layout as an option, but it doesn't seem to cover all the 
updating functions that ctrl-u covers.

Because you have turned Automatic Update Layout OFF. There are many 
good reasons to turn this setting off, but you have to remember to update 
the layout yourself when needed.
