Hi Victor,
 
I am coming in late on this conversation regarding billing of 88342.  You can 
only bill a single specimen once for an antibody. If you were performing 
multiple antibodies then you can charge 88342 x the number of antibodies.  If 
you charge more it can be considered fraud so be careful. A great publication 
for these types of questions is called Coding Alert.  It talks about all these 
types of issues and new cpt's etc.   One other suggestion that I noted in a 
response is if its a self bill or I should say a client bill is to bill them 
the number of times you stained it  as long as its not insurance. This is also 
fraud. You can not change your billing depending on if a patient's insurance is 
being billed or not.

[Histonet] sentinel lymph node melanoma protocols

Good morning everyone,
 
I am curious to see what the standard protocol is on sentinel lymph node for 
melanomas.
I need to establish a protocol at my institution and I have been reading a lot 
of literature and it seems to be a wide variety of thinking out there from 
bread loafing the lymph node first then cutting multiple HE sections and IHC 
in between to bivalving.  Some of the protocols call for up to as many as 46 
HE sections at 3 microns but it doesn't say where to take the sections for the 
IHC (S100, pan-melan cocktail).  Would any of you like to share your protocols?

[Histonet] Follicular staining of Melan A in lymph nodes

Good afternoon everyone,
 
I am having a strange staining pattern that I have not seen before and I was 
hoping someone in histo land may be able to help. I am seeing Melan A staining 
in the follicles only  in a sentinel node. There is no background and the 
staining is cytoplasmic.  The node is actually negative for tumor and the 
staining is restricted to the follicles. I have looked online for any articles 
that may explain this but I haven't seen anything that would explain.

laboratory validation standards to be completed on a stain or
protocol/process developed by another entity other than the end-user. 
Commercial batches are manufactured in such large quantities, that a
reduction in lot to lot variation makes validation more accurate and
allows for accurate reproducibility of the stain process, regardless of
which end user is performing the staining process: the human and/or the
analyzer.

Talk Title: Hard, Harder and Hardest:  Choosing the right process for
your bone project
Abstract: Processing tissue containing bone and cartilage has
challenged histologists throughout the years. It is our responsibility
to understand these challenges, know the tissue and cell components and
provide the pathologist and/or investigator with quality stained slides.
 In recent years digital pathology has created new opportunities for
routine labs to incorporate different processing methods for bone.  In
this workshop we will discuss decalcified (paraffin and frozen) and
undecalcified (methyl methacrylate and Epon) methods for processing bone
that best fits the diagnosis and or analysis.  Detailed protocols will
be presented pertaining to processing and staining of samples containing
bone and cartilage.  We will discuss the common problems with both
methods and share some of the “tricks of the trade”.  We will focus on
aiding pathologists and/or investigators with understanding diseases
such as rheumatoid arthritis, osteoarthritis, renal osteodystrophy and
osteoporosis.  Upon completion participants will 1) identify tissue
components and cells 2) identify which processing procedure best fits
the diagnosis or analysis 3) identify the staining differences in each
processing methods 4) identify artifacts in each method.

If mistakes occur, the lab protocols must be altered to prevent the mistakes. 

If personnel can not or will not follow protocol to eliminate mistakes, then 
employment must be terminated. 

This is not being picky or unreasonable - ask any potential cancer patient 
relying on an accurate diagnosis.

Currrently I am working in a lab where I am the only tech., but in 
previous labs when mislabeling happened, the tech would be written up 
for it after one formal verbal warning. If they were written up 3 times 
in 6 months, then a suspension was possible (although never happens), 
then if the problem continued could lead to being fired. So, basically 
you would have to mislabel something or another 6 times in one year to 
be fired. 
Again...just my opinion...

[Histonet] ink on film coverslips

Hi everyone,
I am trying to completely remove the dotting ink from our slides. I have tried 
alcohol and it takes off the blue dotting ink but the green ink leaves a yellow 
faded dot on the cover slip. The cover slips that we use are the film coverslip 
so I think the ink gets absorbed somehow. Do you have any ideas that may take 
the ink off of the cover slips? 
I would like to avoid taking the coverslip off the slide if possible.

[Histonet] Clo Test CPT

I am looking to see what CPT code everyone is using for reading Clo Tests in 
the pathology department. I have heard of using 87081 but I am not sure if this 
is accurate as this is for culture and the CLO is biochemical reaction not a 
culture.  Currently I have been using 88300 gross only.
Any help would be appreciated.

[Histonet] bone marrow fixative

Good afternoon everyone,
 
In past years our lab used B-5 fixative on bone marrow biopsies for better 
immunostaining.
With the chemical hazards of B-5 we switched to AZF fixative and have been 
using this for the past few years. I am just curious what everyone else is 
using for fixative on their bone marrows and how is your immunoreactivity?  
Maybe a better question is what is the preferred fixative for bone marrows?

I have a Leica Bond and I have had a couple of small issues but I think
that is par for the course with any instrument. Nothing is full proof. I
run a small lab that processes 20,000 surgicals a year. The Bond seemed
more economical for my situation without compromising quality. I have
had the instrument for 1.5 years and I would purchase another.

Oh, I forgot to mention we do this after we face the block.  We put a
little swirl on the block face and let it soak for 10-30+ minutes
depending on spec.  You can usually get a ribbon, then you have to
re-soak for another ribbon.  I hope this helps!

We are trying to cut fish scales that have been decalcified.  They are

chunking out when we try to cut them.  I think we need to soften the

keratin and I looked in the archives for the right dilution of
ammonium

hydroxide to use.  One post said 5% the other said straight.  What

dilution do you recommend?  I'm think it would probably be the same as

what you use on toenails.
