If you're unable to press the down arrow when you hear untitled attachment, try this: open the e-mail and press shift tab once. You should hear the word attachments. Press enter. Then arrow right. Each time you arrow right you should hear a new attachment. Press enter on each attachment to open them. I hope this helps/works.
