I am currently the ward clerk 
in the English ward in Taiwan.  We are still submitting tithing and 
membership related issues by hand to the Taiwan Church records office.  
They have two people there that manage all the units for the island.  The 
word is Hong Kong has been pushing it out to the units there as a test.  
Taiwan should begin to bring it to each unit here by the beginning of next 
year.  I know of at least one other branch clerk in the Asian area 
that is using Filemaker to manage his unit's membership records and 
tithing.

As I said before, the ward I attended in Holland used an Excel
spreadsheet for tithing ;).
It should be fairly easy for your brother to do this in Excel.

I think the main focus is to track tithing donations easier, but
membership is a concern as well. I don't know the details of why he
can't use MLS, I don't think there is anywhere close that would be
economical to sync with LDS headquarters.

As far as I know only the English speaking countries use it. I have been in
multiple countries across the world and I only know for sure that the US, 
UK and Australia use it.

I know that the Netherlands, Belgium and Germany don't use it. And as far as
I understood the main reason was the synchronization with SLC wasn't legally
settled. But then again that was a couple of years ago. Maybe that has 
changed now. I sincerely hope so for the ward clerks ;). Trust me I know 
how it feels to do it by hand!

Any accounting package can do the tithing side.  Just setup all of the members
as customers and record all of their donations as sales receipts.

QuickBooks comes to mind as the simplest to use COTS software product.
