Ok, thanks for the info Christian ….. 
In the mean time I downloaded a new Ubuntu 16.04 s390x ISO file from 
http://cdimage.ubuntu.com/releases/16.04/release/  
I did a clean install using this ISO file and everything installed quickly and 
the IPL ran OK.  So I am able to continue …..

thanks for your offer to help!

Fortunately the root cause is already identified as a bug in zipl (package
s390-tools).
If it triggers depends a lot on sizes/offsets - due to that it was there a
long time undetected - theoretically you could have run into it before or
even with other distributions by bad luck as well.
So IBM already identified the issue (see Stefans mail on the thread a while
ago) and in the meantime provided a fix - thanks to them!

This fix is already integrated in the Ubuntu development release, but yet
in Xenial.
Please see https://bugs.launchpad.net/bugs/1592990 for a recent status and
links on how the fixes go into more Ubuntu releases.

At ICU we have recently bought a LinuxONE Rockhopper (used in combination with 
V7000 so no ECKD) and we are running both zVM and zKVM.   I have been 
experimenting with both SUSE (SLES11SP4 / SLES12), Fedora (23) and Ubuntu 
(16.04 beta) releases and they all seem to work fine.  But on moving to the 
latest Ubuntu release I am seeing the exact same behavior (in zVM) as described 
by Terry. 

I installed the Ubuntu beta version a couple of months ago and has been running 
without any problems ….  When the final Ubuntu release came available I 
downloaded the latest version to replace the beta version. I was able to 
install it in the zVM environment without any issues but on IPL of the new 
virtual guest, I get the exact same problem as Terry describes:  Start 
subchannel failed and disabled wait….  I also ran the trace that was suggested 
and  I get the exact same results as described by Terry in an earlier post. 

Strange thing is that when I tried to install the same Ubuntu version as a 
virtual guest within the zKVM environment everything installs and boots  OK….

If there is anything I can do to provide you with additional information, 
please let me know.

The UBUISO EXEC in "The Ubuntu Mainframe Cookbook" creates a TDISK, copies
the kernel, parmfile and RAMdisk from an install server and punches them to
the reader from there. So no 191 disk is needed.

varies per guest, and UBUNTU EXEC is represented by NORDAUTO EXEC and
CURL REXX. which do not change from Linux to Linux.

The EXEC and "gem" are in a VMARC at ...

Hi Terry,

after some debugging I found a bug in the FBA loader that seems to occur
randomly. But in fact it depends on the size of the the kernel image
and/or some offsets within it. So some kernel images might hit it, most
of them do not.
The image vmlinuz-4.4.0-24-generic I used for a reproduction has an
offset within the file that triggers the bug.
Unfortunately I am not able to recommend a workaround beside using a
different kernel image (vmlinuz-4.4.0-21-generic worked for me).

We already created a reverse mirror of launchpad 1592990. I will attach
a patch to it after I did some further testing.

I do not know where the CCW 0x66 comes from. The bootloader only uses 0x06, 
0x41, 0x42, 0x43 and 0x63 for FBA.
Also I was unable to reproduce this issue on my system but one of our tester 
reported he was able to do so.
I will try to investigate what happens.
