Just play the CD and record the output with a program like Sound Forge or
another recorder program.

markers. I've tried using some CDtoMP3 programs but they seem to insist on
seperating the tracks.

Well, if you can't find a program, you can always rip the CD to WAV files, 
paste them together with something like cool edit, en then encode the 
complete WAV, it's a bit of a hassle but it works, can't say I know of a 
program that converts a whole cd to one MP3...

Rip all the tracks individually and then merge them together using
Soundforge or Cooledit Pro.

The easiest way I know to extract multiple tracks as a contiguous WAV
is to use "cdparanoia" or "cdda2wav" under Linux.  Since these are
open sourced, you may be able to find versions to run on your machine,
or port the software yourself.
