[ozmidwifery] gallbladder

2003-02-04 Thread Rhonda

I am going in tomorrow to have my gallbladder out.
It seems to have gone so fast.
I have spoken to the dr and the anaesthetist about not over dosing me
on painkillers etc so that I may come home tomorrow night and they have
been very good about it. So it may only be a day stay if all goes
well otherwise I will be staying overnight.
I guess I will be sore for a few days but have been assur4ed that
keyhole surgury is not that bad if all goes well.

Will let you know how I go later.
Thanks to all those who offered advise.

  Rhonda

PS: having a fat free diet since Christmas has been gr4eat -
now almost 10kgs latter i have made some really nice low fat meals and
  feel great!
So somethiong good has come out of this after all. i am sure
the weight loss has been helped by the "fear" factor - if I eat it it may
hurt! so I will go without. LOL No cheating on this diet, best
diet I have ever been on. *grin* Not that i would recommend
  gallstones to anyone.
I did test myself and had a few bites of my son's home made hamburger
(not high fat really) Spent a few hours in pain and decided that I would
not do that again. So I guess it is not going to settle down without
  surgery.

Yes Julie this has been sent to Jackie and she and
I are working with a number of others to get things happening (or not happening
as the case may be). I know the press is a mighty tool but don't feel it
is appropriate YET. I am just very cautious about how the press can
manipulate. All we need is for them to end a story with a doctor in a
white coat saying something like "if we don't do this type of research babies
will die" or some other type of crap like that!
I am urging EVERYONE to write their opinions of how
destructive a trial of this type is; how detrimental to women it is and how
unnecessary it is. If you could send them to me I|I will put them with my
letter of concern being submitted to the ethics committees
involved.

  Hi everyone,

I am back with a newly polished
soap box and a major thing to be yelling
  about!

I am going to jump straight into
it with the stress that I am not going to stay on the list for long (a few
weeks perhaps) so you may have to contact me off list for further details.

  Okay, everyone sitting
  down???

  Guess who is doing an
Australian/New Zealand multi-centred study called ACTOBAC A Controlled Trial
Of Birth After Caesarean which (wait for it) is a randomised controlled trial
involving 2000 women. The study is being conducted by Professor Caroline
Crowther. It involves women with one previous cs and they will be
randomised at 34 weeks. Those who are designated the vbac option will
have mandatory continuous monitoring and all the other woman unfriendly
policies imposed on vbacs, inductions or augmentationswill not be
excluded.Theunfortunate women who are allocated to the cs group,
if they (inconveniently) go into spontaneous labour theywill be given an
  emergency cs.

How do I know all this??
CARES was contacted after Crowther read my article in Birth Perinatal Issues
Journal (Sept 2002) and asked us to make some comments on the trialwe made
comments alright!5 pages worth for starters. The issues and
criticisms are many and varied. the language used is appalling, the
information biased and the methodology dubious. An then there is the
little issue of it being completely unethical!we got a reply after
almost three months with a "thanks for your feed back. We are proud to
announce the trial has been approved for
  funding".

  this is an unacceptable and
insulting response to say the least. we have had no further comment from
  them.

So how annoyed are you so far?? it
gets better. As this is a long posting as it is, (typical for me huh?)
If you want to know more please let me know. To end this email I will
give you the first paragraph from the information pamphlet provided to us by
  the research team in November:

"For someone like yourself who has
had a previous caesarean section a decision needs to be made as to whether
your baby is best born by allowing a vaginal birth or whether a repeat
  caesarean section should be undertaken"

  and it gets so much
  better

  hope to hear from interested
parties as soon as possible as I am writing to the ethics committees and need
as much weight to support our objections. (ACMI National level would be
appreciated if you are listening!) Remember that this is going to
recruiting women from every major women's children's hospital in every
  capital city around this country.

Re: [ozmidwifery] End of Caseload at The Angliss

2003-02-04 Thread Sally Williams

Hi Dawn,

This is really bad news. It's amazing what excuses
they come up with to stop something successful in it's tracks, bit like the tall
poppy syndrome. Midwifery at Monash is going through difficulties as well as the
powers that be want to cut our maternity beds from 47 to 32, without capping our
bookings of course. That also means shedding approx 22 EFT!! Who said there is a
shortage of midwives?

WE do do team midwifery at Clayton, but it
doesn't really work very well, the team midwives find themselves working mainly
in delivery suite to cope with lack of num, bers(!) so the women really don't get
the continuity of carer that this model of care is supposed to
create.

Hope you manage to find something that
works

like caseload (which no one desputes is the best model) in a diluted
form to all women instead of the elite few!! but nothing in the way of a
new model is yet proposed negotiations between hospital, Doctors and Midwives
are yet to begin, and a team aproach is whats being put forward, Hence my long
winded email, can those of you in teams please let me know the good and bad

Lesia,

Yes you are right I wasn't commenting on Claudia paying for her birth in the
UK. You made a comment about supporting all choice and you then replied to
say you include my choice as well.

This is great, but I don't have it!

I understand that we must not vent our frustrations on women that choose
medicalised birth (I just feel sorry for them and their bub, btw did you see
that poor bub being pulled from its Mother in the US on Discovery Channel
last night, it nearly made me vomit, and then the health worker say how
great she felt being around birth! Sad stuff, this wasn't birth, this was
production line abuse!)

What makes me so mad is that international best practice care is so much
cheaper but I still can't access it!!! Yes we all know why power and control
etc.

I vote (as do many others) for the NZ 'birth payment' to allow women to
truly decide. Then I couldn't care less who has a elec c/s or births from
their toe!!!

What will then happen is what happens in NZ, women will flock to midwife
care (differing birth locations but midwife care) because even as MC an
others continue to elevate the midwife, she is largely invisible, women out
their have no idea they could have 1 to 1 care and form a relationship with
I carer! Sadly some midwives think the same, and brand our work as
promoting Rolls Royce care. To put the record straight, you bet it's
Rolls Royce Care but it's achievable on a Holden Budget.

2003-02-04 Thread Marilyn Kleidon
Justine:

I think the problem is the managers and accountants don't believe it is
achievable on a holden budget. I think perhaps it is due to us not reducing
women to widgets (which I think is commendable). Somehow we have to break
through the economic wall of silence. I think since NZ is doing it, then the
maternity budget of a health service area there needs to be studied and
compared with a similar health service area here perhaps, I don't know.
Having once been married to an accountant/economist (micro-economist) I do
remember you have to be talking about the same thing when arguing with them
and also it is very easy to be accused of comparing apples and oranges and
that will be that.

You are a midwife, assisting at someone
else's birth. Do good without show or fuss. Facilitate what is happening
rather than what you think ought to be happening. If you must take the lead,
lead so that the mother is helped, yet still free and in charge. When the baby
is born, the mother will rightly say: We did it ourselves!

Re: [ozmidwifery] Thrush handout/gentian violet

2003-02-04 Thread Aviva Sheb'a

Hi, Joy,
I don't know what system you use, but
have a go selecting what you want, then hold 'control' while you press 'c'
(copy) then put your cursor where you want the text to go to, then hold
'control' while you press 'v'. In short, 'cntrl, c, cntrol, v'. Failing that, do
you have a toolbar with a scissors (cut) button, a copy button, and a paste
button? I just about had a party the day I finally worked out the cutandpaste
shortcuts. So easy! for a writer, it's manna from heaven.
Good luck!

Subject: [ozmidwifery] Thrush handout/gentian violet

I can't seem to manage to copy and paste, so will rewrite this handout
here. This is certainly not a definitive treatment and I usually suggest
thatwomen use the parts that they feel they can manage. Also, as
someone elsesuggested, thrush can be very resistant and take much longer to
get out ofthe system. (It is probably time I reviewed the handout
anyway.)Re the gentian violet - this is still recommended by a number
ofbreastfeeding experts. However, it can no longer be purchased in Aus
to thebest of my knowledge. The study showed that it was
carcinogenic/toxic inrats in very large doses, much more than we would ever
use. For furtherinformation check out (Dr) Jack Newman's web
pages. He has heaps of goodinformation. You should be able to
find your way to his articles by goingto http://users.erols.com/cindyrn/printnotenough.htm
or do a search for DrJack Newman. Also Dr Lisa Amir of Richmond
(Melbourne) is something of a "thrush expert".Treatment for Thrush
(Yeast Infections) in the Breast.1. Treat other thrush infections
(vaginal, partner, baby's bottom, etc.)2. Use oral Daktaren Gel on
nipples and in baby's mouth 3-4 times a day.3. Take
acidophilus/bifidus supplement - 6 capsules spaced evenlythroughout the
day. Keep in the fridge.4. Eat a lot of garlic or take garlic
capsules, as it is anti-fungal andboosts the immune system.5. Take
zinc supplement as per directions on the bottle.6. Reduce sugar to a
minimum and use no artificial sweeteners.7. Decrease intake of yeasts
- bread, wine, Vegemite, Promite, biscuits,mushrooms.8. No dried
fruits, grapes, rockmelon or peanuts.9. Good foods to eat include
plain acidophilus yoghurt, pasta, rice, ricecakes, fresh fruit and vegies,
yeast-free bread, crumpets.10. Hygiene - dry upper body and breasts
with separate handtowel or nappy;use soap for hands and nails after nappy
change/nipple treatment; soak brasand breastpads separately, wash, then dry
in sun; boil all feeding aids for10 mins. If using sterilising
solution, ensure that all items are rinsedwith freshly boiled water prior to
use.11. Rinse nipples with 1 teaspon carb soda to 1 cup boiled water
or 1tablespoon white vinegar to 1 cup of boiled water 3-4 times a day.
Pat dryand apply antifungal cream. Air the nipples as much as
possible.12. Continue treatment for at least 7 days after symptoms
have gone.13. If the infection is resistant to these treatments, you
may need to seeyour doctor for oral medication. Kenacomb ointment may
also be useful(script required) - no need to rinse off before feeds as very
littleabsorbed through baby's gut.Joy Cocks RN (Div 1) RM CBE

2003-02-04 Thread Jo Dean Bainbridge
Title: Re: [C-Aware] Randomised Controlled Trial on vbac v's cs (long)

Thank you Gail for your words of
support.
I can not stress hard enough
though how I don't want this to go to the media YET!

I think the media is a
tool that we can use but as a last resort: for e.g. if the trial goes ahead to
use it to warn women against being involved. What I am doing today is
writing a letter to the research team one last time to ask them to confirm a few
things. Even if they don't respond (which will be to their detriment) I am
then going to take it to the ethics committee that has approved the preliminary
trail at Adelaide's Women Children's hospital. I want to hit the
ethics committee with every single argument voiced by as many as possible...so I
need people to write letters of concern and get them to me within the next 2
weeks PLEASE!!!
In the mean time I am meeting with
the heads of OB and Mid at WCH and also bringing this up at the ACMI SA
meeting.
I need as many people as possible to
really highlight just how unnecessary and unethical, uneconomical, etc this
trial is.
send them to PO Box 1013 Nairne 5252
SA
I am scanning the brochures and
placing them on the CARES web site and also going to see if I can get them on
the Birthrites and MC web sites also. I am slightly concerned that
they might have changed the information contained in the brochures (after our
scathing review) but as they have not supplied up dated ones (after repeated
requests) the information I have must be considered correct.
Those who want them sooner please
email me and I will send hard copies. I am writing a summary of the trial
and our concerns. Anyone who wants to access it can print it in their respective
newsletters or journals.
cheers for now

I agree with Jackie, Jo. You're definitely in the right in
this instance. I had an opportunity to have a fairly close read of both
brochures last night, and gave it to Neil to read, too. We were both pretty
shocked by some of the info in them, esp. the one for prospective
participants. Some of the info is simply untrue! Their take on the risks, for
example: "For a woman who has a vaginal birth after a previous caesarean birth
there is a very small risk (1 in 150 or 1 in 250 is a very small risk
according to the brochure) of the uterus tearing leading to serious problems
for the mother, such as major bleeding and needing a hysterectomy" If anyone
can correct me please do, but I thought the risk of rupture during VBAC was 2
in 1000? Maybe I've got my info wrong?
I'm not even going to comment on the ethics of 'randomising'
(which is a vastly different word to 'random' with a completely different
meaning BTW) 2000 women into two groups! There's been plenty said about that
  already.
I don't think your concern with this study or your actions
against it are alarmist or hysterical (which is how the media usually portray
those of us without the dangly bits when it comes to natural birth). From
what I can see you've strategised your plan of attack perfectly and are going
about it in a well ordered and competent manner.
Best of luck with it on your end.
  gail.

I am very protective of
CARES reputation and do not want to see it fall with the trial (if you get
my drift).Understandably, Jo, thats why every piece of
information that your group passes on to the bigger birthing community needs
to be checked and double-checked to be accurate whether thats by email or
through the media. As long as what you are saying is true, then any mud
thrown just wont stick. Okay? Dont let the big guys scare you, its all a
game to them.Take care,Birthing Beautifully,Jackie M.

I am going in tomorrow to have my gallbladder out.
It seems to have gone so fast.
I have spoken to the dr and the anaesthetist about not over dosing
me on painkillers etc so that I may come home tomorrow night and they
have been very good about it. So it may only be a day stay if all
goes well otherwise I will be staying overnight.
I guess I will be sore for a few days but have been assur4ed that
keyhole surgury is not that bad if all goes well.

Will let you know how I go later.
Thanks to all those who offered advise.

Rhonda

PS: having a fat free diet since Christmas has been gr4eat -
now almost 10kgs latter i have made some really nice low fat meals and
feel great!
So somethiong good has come out of this after all. i am sure
the weight loss has been helped by the "fear" factor - if I eat it it
may hurt! so I will go without. LOL No cheating on this
diet, best diet I have ever been on. *grin* Not that i would
recommend gallstones to anyone.
I did test myself and had a few bites of my son's home made
hamburger (not high fat really) Spent a few hours in pain and decided
that I would not do that again. So I guess it is not going to
settle down without surgery.

No because the sound of financial benefits (like
the US saying "of course we will buy your wheat! BULLST) will always ring
louder!!!
I have heard that the Lord Mayor of Brisbane is
pleading with his constitutes to return the "How to Spot a Terrorist" $15, 000,
000 booklet back to the commonwealth government. I think it is a great
idea, and as Downer is our 'local' (yeah right) I will certainly be sending it
back with a note saying how $15,000,000 would be better off spent implementing
NMAP !!!
I encourage others to do the same.

I am interested in reading the 'how to spot a
