Debby,
Qld is very proficient in cost shifting - State to Federal. 
Qld has suffered from its low tax state since GST came in. Tax credits went more 
to other states when the GST came in because they had to remove some of their 
taxes thereby loosing some income which the Fed compensated them with. 'Cos Qld 
didn't have many taxes equates to minimal increase in Federal revenue over 
several years but we can build great football stadiums when we are broke! State 
funds are then divided up, unfortunately health has not featured very much in 
recent state budgets, most have gone into family and education initiatives i.e. 
new programmes.
The reduction in the number of Drs bulk billing has had an 
impact on our antenatal clinic numbers. Ladies are electing to come to hospital 
ANC rather than share caring with the GP paying a $15-20 gap. Handy for us 
midwives in the ANC to develop a positive relationship with birthing women and 
their family an enhance midwifery knowledge.
 
In Qld Health, funds are allocated into each health 
district then it is up to the district manager how the funds are utilised- the 
health district budget. They look at all sources of funding to fund many 
programmes. One example is the Federal health initiative on diabetes and asthma. 
Immunisation incentives i.e. money are given to GP's if a child is fully 
immunised. This is one of my pet hates- drug company bribes. Private midwives 
need Medicare provider numbers in order to have admitting rights due to the DRG 
costings of the Federal health system. Gets too complicated after that to 
explain.
 
Nursing/midwifery staff costs are a large part of any budget 
and also the one that gets cut the most - why one may ask- because we take it!! 
When will nurses and midwives stand up for themselves and ultimately for their 
clients. It is us who has to face the QNC when complaints or allegations are 
made. If we have insufficient staff in Birth Suite how many midwives would 
complain and follow up with an incident report. I do and now I am classed as 
been 'difficult' because nobody else complains. Why can't we be nice to each 
other.
 
My role as a health district consultative forum Union rep is 
one that questions these budgets, workloads and other member related issues. I 
am an activist not a couch potato waiting for action from others to do things 
and often these people are the first to whinge when they are 
affected.
 
As for community cabinets, this is an initiative of Peter 
Beattie's cabinet. A location is decided, usually in response to interested 
members of the community simply writing to the Premiers dept to be 
considered . I think it is held each 3 months and all ministers and usually the 
various Directors- General attend. Any member of the public can request an 
'audience' usually 10 - 20 minutes. People can present their views or requests, 
notes are taken by staffers and feedback is usually made within the month. Very 
effective lobbying forum. You sit across the table and push your 
issue.
 
In the ALP we also have community forums such as the one 
Sandra  (Mareeba) mentioned. Change occurs because members raise issues and 
seek change. NMAP is slowly gathering momentum in the ALP and even in my faction 
( yes we do need this forum as well) people even searched me out, knowing I was 
a midwife, to ask me about 'this stink about maternity services' they had heard 
about.  NMAP is great and in this ALP forum I am able to state my 
personal views as a midwife (not restricted by my membership to a midwifery 
organization). They leave enlightened and ready to pass on their view to 
others.
 
Sorry its such a lengthy response but health care is not an 
easy system to explain.

be a great place to lend an ear to some influential political members and 
members of the community about NMAP. Maybe even Advance Australia where... 
, this could be birthing reform. All about networking, being seen and heard and 
preferably not always the same people.
Any Sunshine Coast midwife available to attend. I am a bit 
concerned about the time though 12am - 3 but it did say lunch.

Some members of the ALP are midwives and are 
active members of ACMI and the ALP.  I am aware that the Brisbane 
branch of Emily's List has received a copy of the NMAP. I am aware that QNU has 
a copy of the NMAP and members of the ALP Queensland health policy committee 
also have the NMAP. 
I can only speak for Queensland in areas I know. 
I don't see many midwives in these forums, where are 
they?

The agencies will not provide PI for midwives to work in Birth 
Suites, therefore most hospitals are not putting agency midwives there. 

At the recent QNU conference a motion was passed for the QNU 
to investigate this practice. many ward based midwives are very uncomfortable 
going to BS as directed by their employers as many have not been in there for 
years-sad.

I am employed by a public hospital and yes it is a condition 
of employment but nobody asked me for proof of my immune status when I started. 
How do they check? I am also allergic to Tet Tox so that's another one I'm not 
protected for.
If I have a needlestick/splash I get tested privately as I 
believe the employer is not out there to protect the worker rights! I do fill 
out incident reports if I am splashed/jabbed.  I do follow my 
employers guidelines for immunizations but I do stress for parents to do their 
own research before they make any decision. If they decide to vaccinate so be it 
I will do so.
 
 I had been vaccinated when the synthetic Hep B became 
available 1987-88 when I was working in ICU. I assumed I was OK. 7 years later I 
found out I had no protection to Hep B. 
 
I am personally against universal Hep B vaccinations for 
various reasons, none more stronger than the link with Chronic Fatigue and liver 
failure.

further to the 12 hour shift

To all, 
I hope this email doesn't result in more hate mail and virus 
poisoned posts which seems to occur when one posts differing views on this list 
but in view of Judy's posting of my initial reply I felt it was also necessary 
to post my response to balance the debate and I have also asked some other 
questions
 
To the aspiring midwives who inquire about pay rates may I 
humbly suggest contacting the various registration boards and the associated 
state, dare I say it, Nursing unions around Australia to see exactly where 
'they' view direct entry midwives in relation to pay scales, employment 
opportunities and accountability. Sorry but that's the system in place at 
present.
I have heard whispers from some noted quarters 'not on par 
with registered nurse/midwife but along with enrolled nurse level'  which 
is rather unkind. 
 
As various nursing/midwives conference motion agenda items are 
being prepared may I also suggest midwives specifically direct any midwifery 
related agenda items to be discussed  at the next conference start 
submitting to the local branch of QNU and associated unions. Get 
involved!
One I have already submitted through our local QNU branch is 
along the lines that graduate midwives be supported in the post graduate year 
similar to what Queensland Health presently offers graduate nurses. Presently 
here in the SE Qld we have produced excellent graduate midwives but alas they 
are having trouble gaining employment in metropolitan hospitals which provides 
supported practice and instead are re-entering nursing or working in rural areas 
without support with mid clients few and far between.

participants on the list expire?How many times does the on call midwife

asking is I am the only practicing midwife on the Industrial Policy 
committee of the QNU which has looked at several applications for 12 hour 
shifts in many areas but each QNU conference the submissions get voted down 
by members. We are currently reviewing a submission made last conference 
where after working 12 hours or more nurse/midwives will be required to have 
a full 24 hours off to prevent fatigue. What is your opinion of 
this?Recently, I saw an application to the SBU that QH wanted to close the 
McKay birthing teams project as quote 'it wasn't meeting clients needs'. Do 
you know anything about that?
