The doc who did my tendon transfer also put a pin in my thumb and really
tightened my index finger so that my thumb would supposedly pinch against
the index finger.  The pinch never really occurred.  Instead the straight
thumb just got in the way, the index finger was too curled and the pin
broke.  Yeah ouch.  So I had a surgery to correct that problem and now my
index finger is still pretty tight.  Not all the way against my hand or
anything, but the stitching went up the inside of my finger so there is scar
tissue there.  The pinch works in a way, but not the way the doc thought it
would.  I need that thumb to bend.

Now I just have a really strong grip and it allows me to grab something with
one hand when I bring my wrist back.  I was able to open my hand a little
more and i lost that and now my hand is a little tighter, but for me it was
worth it.  I used to use my left hand to shake hands with others because I
could open it more but now that index finger gets in the way.  Oh well, I
can hold a bottle of beer at the neck with no problem now!  Like I really
need to ;-)

Tendon transfers, are nothing new.  The procedure has been around for a
couple of decades, with a goal to restore function to its candidates.  Its
not something you chose like a loaf of bread.  Those wishing, must be
evaluated to determine that they are good candidates for the procedure.
Goals have to be established too.
Most who have had the procedure claim that it has improved their lifestyle
and survival skills.

I was faced with the same dilemma.  I was running Dragon  Naturally Speaking 
8.0, when I encounter a problem with it on my computer.   I could not get it 
to work properly.  I deleted it from the computer and  tried to reinstall it.  
It didn't work.  I purchased a 9.0 version,  only to find that my computer did 
not have enough RAM, and the processor wasn't  fast enough.  My husband 
looked into purchasing more RAM, but by the time  he came back from the store, 
he 
had just purchased a new computer.  Of  course, I had no problem loading the 
9.0 on this computer.  Both computers  are laptop computers.
 
I guess this isn't much information on answering your  question, but I was 
just offering my experience.

I’m so sorry to hear about your Medicaid problem. It is so difficult to find
and keep good CNAs! I’ve had one private attendant for six years, and even
though she has her problems, it’s great that she knows me well. My other two
attendants had been with me for about a year, so I’m for the shoe to drop,
because a year is about all they last with the agency. By then they realize
they can make more money privately.

I am a C4, so I don’t know if I would be eligible for tendon transfers. But
I’m going to talk with my SCI Dr next week about it. Joan

I am a C4-5 complete, I have no trunk control, I feel like a ragdoll!!

Oh my, yesterday my Nursing Service had a manditory meeting with all of
their home health aids. This makes me sick, They sent no letters, although I
talked with Jorgiene the owner yest & she said that I should have gotten 3
letters. My cna & I do mail everyday, we pick it up then open after dinner.
Anyway, my case manager has tried very hard to get me on Medicaid Waiver &
finnally last fall. she got me on it. Well our N.Service is dropping
Medicaid Waiver, claming they are several months on payment from Waiver. She
said,  she has lost thousands of dollars. Well it's our crummy governer
Mitch that has changed the laws. We in IN have to get busy & ditch Mitch out
of office!  They can give $900 million to build a new stadium but they
couldn't give funding for all-day kindergarten. It's  a rotten shame, where
these politicians heads are! Anyway back to me, My CNA's can no longer take
me to my Dr's appt., cannot take me out shopping or pick up my meds from my
pharmacy. I save the government money, by having Liz take me in my vehicle &
use my gas, during the time Liz is here to take me. I'm sick about this,
sorry guys I just had to vent!

  Here is some info on the anti-seizure drug phenytoin (Dilantin).  I think you 
might want to ask your doctor about some of the newer anti-seizure drugs, like 
Gabapentin (Neurontin).  It has been used by some on the Quad-List with pretty 
good success.  I hope this helps.  Stay strong.  
   
  Side effects limit treatment  Anti-seizure drugs have been used to treat 
nerve pain for more than 50 years, but their use is limited by the severity of 
side effects they produce.
  Older anti-seizure drugs — carbamazepine, phenytoin (Dilantin) and valproic 
acid (Depakene) — can damage the liver. Other side effects include rash, 
nausea, vomiting, double vision, loss of coordination, drowsiness and headache.
  Because these older drugs generally have more side effects than do the newer 
anticonvulsants, they often are used only when the newer medications prove 
ineffective. Despite the fact that they're older, they can be at least as 
effective as the newer drugs for pain relief.
  Newer anti-seizure drugs  Many new anti-seizure drugs have been developed in 
the past 15 years. Some have proved effective for relieving the pain caused by 
damaged nerves.

   Pregabalin (Lyrica)

The reason I'm writing in now is because they were serious and I am hoping 
that my experience can help prevent some other quads from having to go 
through what I did. Basically what ended up happening was that those 
episodes kept happening until one day, what I feared most - an occurrence 
happened where sitting up did not alleviate the symptoms. It became obvious 
that I need to go to the hospital. On the way there I could feel that I was 
not going to make it. My Dad was driving and I was telling him what my 
symptoms where and that I was about to leave. He helped me by just telling 
me to let go and to not be afraid. My eyes rolled back into my head as I 
reclined the wheelchair. Everything turned white. I woke up about 4 days 
later in the hospital. Apparently I had had a seizure. It actually, now 
looking back on it (it happened on Feb 19th) it has been an interesting 
experience. When I started becoming aware again I couldn't remember the 
things that had happened right after I lost consciousness. I also lost 
memory of many trivial things like the scenery around where I live, but 
that soon came back to me. The experience has changed my perception of 
things a bit. I can't say that being confined to this body in this 
wheelchair makes 'sense' to me now, but in a way I'm more able to observe 
everything more and see the layering behind what we see through our eyes. 
So that there are explanations for things as they are. Anyway I won't get 
too weird here :-)

What I really wanted to say, but got a bit side-tracked, was that the I 
found out the reason why it happened. At least this is what the ER docs 
told me, and what kind of upsets me is that I've been telling my docs about 
this for quite awhile and they never clu

I'm a  bit confused myself because all that the doctor told me is what I 
wrote, ie that it had to do with a chemical that I lacked from not standing 
up. I'm going to do some research of my own. I did look up the drug on 
drugs.com and it is a pretty heavy drug indicated for seizures, epileptic 
stuff, even asthma. I don't think that it was bowel or bladder related 
though because those were/are functioning well plus I have actually had a 
few minor of these spasms since I've been home, but they've been much 
weaker and haven't been riyhmic, and my right- stomach area(below the 
rib-cage) is no longer sensitive to touch like it use to be.


It has occurred to me that the doc was just saying something that popped in 
his head, because it was better (for him) than just saying 'I don't know'. 
I mean, it could be any number of things, including genetic (though I'm not 
aware of anyone in my family having seizures). I know that I'm still not 
clear on why it happened.


One thing that made me think that there was something to what the doc told 
me, is that when I had/have the spasm it is a full body spasm and makes my 
body try to straighten itself out and even stand up. It really feels like 
my body wants to stand up and it always makes me feel better to sit/stand 
up as much as possible. I mean, it feels like what would feel best was if I 
could stand up. Anyhow, its going to take me a few weeks, but I'm going to 
talk to the doctors again about it, and I'll let you know what they say.

My neurologist told me that it is a result of having a trauma, after I had  
my first couple seizures.  I started my medication after testing and have  
never had a seizure since.  I take 50 mg Tegratol twice a day.  What  do others 
take? I started medication about two years ago after my few  seizures.

What level are you? I have not considered having this done before, but if
I’m reading the information correctly, it might be possible for me to gain a
little mobility. My joints are very flexible, and I can extend-or have

I had muscle & tendon transfers to bilateral arms in 99' before I moved out
of the Nursing Home, in hopes to be able to feed myself & use a joystick on
my w/c arm. I had both arms done at the same time, which I'm glad that I
did. When I woke from the surgery, I said oh I can't believe I did this
because it hurt like hell!!! The 2 Drs that did the work were good surgeons,
who specializes in arm surgeries. They both work with quadriplegics from
RIC. I did get a lot more movement in my arms but just cannot do my goals.
The Drs cleaned out the calcium from my elbow joints, they could only
straighten my arms out to 40 degrees. If I hadn't tried having the surgery,
I would not have known about being able to use the joystick.{That was my
ultimate goal!}.

I have very good use of my arms while I'm in my wheelchair but I have no  
grasp with either hand but I can flex my left wrist up real well but my right  
wrist is limp. I where a plastic splint on my right wrist to give me  
functionality and now I have, like I said in my last post, a mechanical brace  
that 
gives me grip when I flex my wrist up similar to having tended  transfers.  
Since 
I was able to work, drive and do other stuff with my left  hand I was not 
willing to take a chance of having a failed tended transfer  surgery when I 
could 
get the mechanical brace and have 100% guarantee that it  would make me more 
functional.

There is an alternative to surgery for tended transfers.  I went to  the Mayo 
Clinic in Rochester, Minnesota to see about tended transfers.   They have 
approximately and 80% success rate but if something does not go well  there is 
no 
turning back and you're going to lose what you already have, which  in my 
case is not much.  Instead of taking the chance of losing what I have  I ended 
up 
getting a mechanical brace that does basically the same thing as  tended 
transfers if you are able to flex your wrist up.  It looks something  like out 
of 
the terminator movies but I couldn't care less about looks because I  like to 
function.
