I suffer the same problem with my PC. It is caused by the Ubuntu insistence in 
using 'pulseaudio' in the new releases. I patched up my PC by removing 
'pulseaudio' and installing 'esound' in its place. Now the Audio works and I 
can use Skype. The problem is that, with  any new update, Ubuntu reinstalls 
'pulseaudio' and I am obliged to repeat the procedure. 
You can find how to do it via Google or ask me for more details.

Following the update suggestion, I upgraded to 9.04. When with 8.10 I was able 
to use the PC in xorg-failsafe mode, the update to 9.04 does not allows me even 
that!.
 At booting, at the time you expect the Desktop screen, a message appears in a 
red window and a black screen 'OUT OF RANGE' and everything stops there.
Now I am in xorg-failsafe but only less than half Desktop is usable.
I know of the files requested for xorg bug reporting, but the only files that I 
can supply, hopefully, are for the xorg-failsafe mode.
My PC is a HP 753 using Intel graphics 82845.
If there is a way to get the other needed files, please let me know and I will 
oblige.

killall pulseaudio
sudo apt-get remove pulseaudio

I want to point out to you, with the hope that this would be of Help to Ubuntu. 
Xorg malfunctions, also, on my other HP 1160 PC AMD64 with Nvidia GeForce 
FX53200XT graphics and Ubuntu 9.04 Release.
As per a previous Bug report, the monitor , a Samsung 193v, is reported as 
Display Unknown and the resolution settings cannot be changed. It seems, 
unfortunately, that, since the Ubuntu 7.10 Release, Xorg misbehaves. The many 
updates, instead of correcting the problems, have made them worse.

After the problems suffered with Alpha and Beta versions of Ubuntu 9.04, I 
downloaded the i386 desktop release version with the hope that Xorg 1.6 this 
time would work on my HP 753.
But I was wrong!.
The installation started fine. Then,a red window appeared:  INVALID MODE.
The HP 753 PC Uses an Intell 845 Video system.
Any hope that some day xorg and pulse Audio will be working in Ubuntu ?.
Why to publish a faulty release of Ubuntu ?.
I cannot supply additional informations, since the 9.04 does not install.

From the long output, obtained with Knoppix 5.1.1 using lspci -vvnn, the 
portion of concern is:

The Error Message is a simple one: INVALID MODE in a small Red Window.

Now, after some of the many updates, the refresh rate stays at 75 Hz and and 
1280x1024 resolution.
I can not, however, set the resolution to any other value without causing a 
shift of the screen to the right and a black strip on the left of the screen.

Package Manager' in order to be able to install, the OpenOffice-base First and 
then the 'Spreadsheet and Word' later, I can tell you only by memory that the 
messages were: Depends on XY packages. Not installed.
I have not been able to locate the apt-cache policy openoffice-base requested 
file.

Printer 'Power On the following message appears:
Missing Printer Driver 'No Printer Drive  for Brother USB Printer.  
Submitting a Test Page for Printing the following message:
Print ERROR, There is a problem processing Document
Enabling the Diagnostics results with Status Message  
'/usb/lib/cups/filter/foomatic-rip failed' .

[Bug 339732] [NEW] Only Max Resolution usable, Monitor Unknown

The PC native resolution is: 1280x1024 pixel and a refresh rate of 75 Hz. The 
Display Settings says: Monitor Unknown. No avail to click on Detect Monitor.  
Any attempt to change the Resolution to a lower value, and/or the refresh rate 
result in a Screen shifted to the Right and a Blank strip to the Left.
I Find that this high resolution makes the characters almost illegible. 
Please, find attached /etc,X11/xorg.conf file. I will follow the 
/var/log/Xorg.0.log file later since, it seems, that only one Attachment at 
the time is accapted.

Now, tanks to some of the updates, I can play again some of the programs with 
sound use. But not Skype.
To use Skype I have to remove 'pulseaudio' and install 'esound'.

Recommends: gnome-games but it is not going to be installed.
Recommends: ubufox but its not to be installed.

I run apt-get -f install, as recommended, but no noticeable effect.

'Brother-HL-1430may not be connected'.
No change  by clicking on 'Printer Configuration-localhost' - Connect.
The 'Document Print Status (my jobs)' shows one file, at least, in the Print 
cue.

I Downloaded and and installed the last Updates Distribution, (28 of February).
Although it contained 8 CUPS related update, my Printer still does not work.
I get a lot of messages like 'Job 47 completed', 'Error BrokenCount 0', 
'Printer Brother-HL-1430 may not be connected, but no printing.

The download and installation of some of the approx. 600 Files made my printer 
useless.
Every time I switch the Printer on, a 'Missing Printer Driver' message appears.
After all searchs for the Brother HL-1435, nor the Test page or other files are 
printed.
Clicking on the crash report Icon results with 'The foomatic.rp closed 
unexpectedly' message.
Previous problem, (see Bug #: 321164), ha been resolved and the Printer worked 
fine up to the installation of the 'Updates'.
The Printer is a Brother HL-1435 laser.
The PC is a HP 1160 with an amd64 processor.

I want add that the settings for the screen Resolution are: 1280x1024.
This is the default setting at installation.
It is the only setting that, when the Refresh Rate is adjusted at 75 Hz, the 
screen is filled and no Black strips are visible.

[Bug 329562] [NEW] Refresh rate does not hold in Ubuntu 9.04

Every time I restart the PC, a Black strip appears to the left of the screen.
A check with SystemPreferencesScreen resolution, shows that the Refresh Rate 
has been reduced at 60 Hz from the previous setting of 75 Hz.
I reset it at 75 Hz, but next restart of the PC, the refresh rate returns again 
at 60 Hz.
The monitor is marked always:  Unknown.
I suspect that it is a Xorg problem.

I am sorry to inform you, that despite several try, I have not been capable to 
run the bash program.
I have been able to download the text but not to transfer it to the Terminal 
for running it. It stays in Firefox and any attempt to coy it for execution to 
the terminal results results with The folder content cannot be displayed 
message.
 
 I believe that the merge of the two bugs makes sense.

yes, I can ear the status sound of my machine. So I can ear the sound from 
'Sound recorder', 'ZynAddSubFX', Skype but not from Rhytmbox.
If it is of any use to you, I want tell you that I had sound problems with this 
machine as reported in Bug# 320063. They suddenly disappeared, perhaps as 
result of some Update.
I just tried to play a CD with Rhytmbox in another PC with AMD64 processor with 
the same result: No Sound.

I want to add that, after sending the previous note, I downloaded and installed 
Amarok.
This program does not work either. At the beginning. after clicking on the 
Icon, a notification of error appears: Signal 11 SIGSEGV.
I tried to understand the meaning of it, but the KDE help system is to much 
complicate for me.

In order to use the Skype application, I have to remove pulseaudio from the 
PC. By doing so I can use Skype, but none of the Audio in the other programs, 
does work,
Reinstalling esound does not help.
Many of the Updates, unfortunately, upset the settings and I can not use Skype 
again.

After changing the Screen Resolution from 800x600 to 1024x768, the status bar 
reappered and now it is fine. It means that I have to use 1024x768 resolution.

I am adding the following note in the hope that it my be of help to the Bug 
investigators.
I installed Ubuntu 8.10 by installing 8.04 First and then upgrading to 8.10 
Intrepid because the installation by CD failed. Using the CD, after the 
preliminary loading of the installation Files, at the start of the Kernel 
loading, the red 'OUT OF RANGE' window appeared and the installation terminated.
With 8.10 Intrepid loaded, I cured the appearance of the Red window by 
restarting in Safe mode and then, as Root, using Nano to add:
HorizSynk 81-101
VertRefresh   60-70 
to the 'Section Monitor' of /X11/xorg.conf.
The PC is now usable, but a bug remains. SystemsPreferenceScreen Resolution 
window shows only
   'Refresh Rate'  0 Hz
I regret not having the knowledge to help in the debugging.
I hope that next Ubuntu release will be less buggy than 8.10.
By the way, why submit us users with the trauma of a new release every six 
months and not to use the
continuous update way as the Linux Kernel does?.

I installed the -intel-gfx-testing... as suggested in the received automatic

I am sorry to report that the red window is as before. Unfortunately, after the 
booting, now the system goes into a loop. To shoot down the PC, I have to 
switch off the power.

Please, find attached the two requested dumps.
One is with booting direct to user settings, ( without username/password ).
The second is with username/password reactivated.
I want to point out to you that the Red 'OUT OF RANGE' window appears in 
both cases.
With 'No Password' settings, the Red window stays on a few seconds and 
then the booting restarts to completion.
With the normal settings, (Username/Password), the Red window stays on. 
By entering of the  Username/Password after the drum beat many times, 
the booting restarts and the process is completed.

Eliminating xorg.conf by changing its name did not have any effect, 
everything behaved the same.

I did try dpkg-reconfigure xserver-xorg with the hope that it would help to 
make my PC operating properly. But this program freezes after the Keyboard

This time I did try to upgrade using gksu 'update-manager -d'. Every thing 
procedded fine until the program loaded.Then, again the  OUT OF RANGE  window 
appeared.
I did try to start in recovery mode and I run all the suggested utilities to 
fix the system, with no avail!.
The followng messages appeared:
X: warning; process set to priority -1 instead of requested priority 0.
Fatal server error: Server is already active for display 0.  If the server is 
no longer running remove /temp/.XO-lock and start again.
Invalid MIT-MAGIC-COOKIE-1 key giving up.
xinit: interrupted system call (erno 4): Unable to connect to X server.
xinit: Nosuch process (erno 3): Server error.
File system check /dev/sdb1: 181804/24723456 files (1.5 % non-contiguos)
 What is OUT OF RANGE that makes my PC unit a scrap of metal. Fortunately the 
Window XP is still working!.

Rhe CD I used is a 32 bit for Ubuntu 8.10, downloaded from Ubuntu web site. The 
CD tested OK with the test supplied on the installation CD. I want to remind 
you, that. I had the same 'OUT OF RANGE' also when I was Upgrading with gksu 
update manager -d.
Fiddling around, ( I do not know exsactly what i did ), using the recovery 
display mode, I have been able to start up Ubuntu 8.10, with a resolution of 
800k600, when with Ubuntu 7.04 I was using 1024x768 and Refresh Rate of 70 HZ.
Now, the only Refresh Rate available is: 0 Hz.

I can not install Ubuntu 8.10. Aftar the language choice, I get 'OUT OF RANGE' 
message. At present I have no problem with Ubuntu 8.04. This happens when 
trying to install from a Downloaded CD.
Similar problem I ecountered when I tryed to 'Upgrade' from 8.04 to 8.10

I am able to open and read the file /proc/bus/usb/devices with Mozilla Firefox 
also. Onli with Nautilus and Gedit i get a blank page.

[Bug 272506] [NEW] Special Proc Files are displyed as empity

Binary package hint: nautilus

When I click on '/proc/bus/usb/devices' to display the content of '/device' 
file, a blank page appears and no content of the '/device' file is displayed.
No problem if I use 'cat /proc/bus/usb/devices. No problem, also, if I open the 
file with VIM or EIMACs editors.
Using 'ls' the file is reported as 0 content  Text type.
I have the same problem if I use gedit editor.
I reported the bug for gedit and it is under rewiew as Bug 262800
