After "apt install gresistor", it failed to start, citing a missing
module for glade:

starts properly. A quick peek at the dependencies using aptitude didn't
show a dependency on Glade:

further suggests that external USB drives /can/ be made to work using
the workaround you described.

My opinion is that adding usb-storage to /etc/modules is an effective,
if unfortunately necessary, workaround. As the issue appears specific to
usb-storage and libusb, I have assigned this bug to the libusb-1.0
module. I'm sorry I can't be of more assistance to you, but lacking an
external hard drive, I am not able to attempt to reproduce this bug.

The take-away here is that the firmware files you need aren't available
at boot time, that is, that they don't appear to be in the initrd file.
This much you can see by saying "mkdir foo; cd foo; zcat
/boot/initrd.img-$(uname -r) | cpio -id" and then looking for the
firmware file in foo/lib/firmware.

I have limited experience with initrfamfs tools, so prudence demands I
defer to someone with more experience. I don't want to recommend a
procedure that will render your server unbootable :)

Okay. It doesn't appear to be a hardware problem. At this point, I
suspect KDE4's compositing or perhaps the underlying Xorg server, but,
honestly, I'm not sure. Maybe other KDE4 users with your hardware and
configuration have experienced similar problems.

Do you have the radeonfb module loaded? What happens if you boot with
"vga=normal" appended to the kernel command line?

I tried to repro this bug in 32-bit 9.04 (rhythmbox 0.12.0-0ubuntu4).
Unfortunately, RB wouldn't freeze. /However/, I get different behavior
that still seems to implicate cross-fading.

With cross-fading enabled:

0) Login.
1) Start RB.
2) Attempt to connect to the reporter's "bad" radio stream 
(http://server-06.stream-server.nl:8080/).

wIth debug symbols installed and cross-fading enabled, the music starts.
The same obtains with cross-fading disabled whilst running under GDB.
The fact that this reliably reproduces (for me, on 9.04) without the
debugging symbols loaded but will not reproduce in a debugger suggests a
race condition.

Yes, you are using the binary NVIDIA driver. From /etc/X11/xorg.conf:

Please disable Compiz: System -> Preferences -> Appearance -> Visual
Effects -> None

Then logout and log back in and retry unminimizing a minimized window.
